This form has been designed to assist Parents / Carers with the Initial and Review Plan Meeting.

Current Medical & Supports is a great tool to assist you in giving a clear view of the participants current situation in conjunction with the assistance of a Medical Practitioner.

This form has been designed through my own personal experiences with the NDIS, I found more specific medical information was required. 

We have come to realise that it is important, whilst having your main Disability registered, that most people have subsequent conditions that affect their ability to function in daily life, and these are not registered with the NDIS.

Providing these are documented medically, they can be also registered as subsequent conditions with the NDIS and this can assist with your funding allocation.

The aim of this form is to allow the Participant of the NDIS, to with the assistance of a Medical Professional give a true and accurate description of the Participants current situation.
