Other types of documentation that maybe of value to the NDIS for submission of an Access Request are:

• School IEP’s
• Reports from non Allied Health Professionals that have been supporting the Participant
• Reports from Support Providers
• Reports form Support Workers
• any documentation that describes or details the Disability/ies of the Participant and how it affects their daily life.
• The NDIS prefers that documentation be no older than 5 years.
