Submission of Access Request

I’m Online Check List assists in insuring that all documentation required is listed so that you can tick off each document, form, etc, when submitting your Access Request.

Only Submit copies of your documents, our recommendation is that you right “copy” at the top of the document so that you can see that is the document your are submitting.

If you are in a location that has an NDIS office we recommend the following:

  • Email to
  • Personally take the documentation to the local NDIS office and submit them
    • Always request a receipt number when doing it this way.
  • 2 Days after submission we recommend you ring the NDIS to confirm receipt of the documentation
