This is for collecting general information about you, how your daily life is impacted by your disability and the current supports and services you receive.

These details can be used to start a conversation about your life when meeting with your ECEI Coordinator, LAC or NDIA Planner.

The Reality is quite often not what it appears to be.  When Planning the type of information you need can range from Doctors Reports to Assessments/Reports and Quotations of expected costs going forward.

If the participant doesn’t have these, then at the meeting the LAC is likely to say that not enough information has been supplied to back up the requests / goals and they will fund Assessments rather than support hours, so that the Participant is able to obtain the level of proof required.
