Reviewing and changing participants’ plans

• Participant may change participant’s statement of goals and aspirations at any time
• A changed version of the participant’s statement of goals and aspirations.  The plan is taken to be replaced by a new plan comprising:
  • the changed version of the participant’s statement of goals and aspirations
  • the statement of participant supports in the existing plan.
• The Agency must provide a copy of the new plan to the participant within 7 days of receiving the changed version of the participant’s statement of goals and aspirations
• A participant may request a review of the participant’s plan at any time
• The CEO must decide whether or not to conduct the review within 14 days after receiving the request
• The CEO may, on the CEO’s initiative, conduct a review of a participant’s plan at any time.
• The CEO must conduct a review of a participant’s plan before the plan’s review date and in the circumstances, if any, specified in the plan.
• If the participant does not wish to change the participant’s statement of goals and aspirations, the statement remains unchanged and forms part of the new plan.

Information and reports for the purposes of reviewing a participant’s plan

• that the participant, or another person, provide information that is reasonably necessary for the purposes of
  reviewing the participant’s plan
• that the participant do either or both of the following:
  • undergo an assessment and provide to the CEO the report, in the approved form, of the person who
    conducts the assessment;
  • undergo, whether or not at a particular place, a medical, psychiatric, psychological or other examination, conducted by an appropriately qualified person, and provide to the CEO the report, in the approved form, of the person who conducts the examination
• If information or reports are provided after the plan is reviewed, the plan can be reviewed again and if necessary replaced
