This support item is for the provision of skills training, advice, assistance with arrangements and orientation to assist a person with disability moving from school to further education.

This support item can be delivered to individual participants or groups of participants subject to the rules set out in this Price Guide. If a support item is delivered to a group of participants then the price limit for each participant is the price limit set out in the table below divided by the number of participants in the group. Providers should make a claim for each participant using the relevant support item. Each claim should be for the total time of the support but is subject to the lower price limit as set out above.
